2994: registrar case 190 is excellent

This video showcases a great job by an anonymous registrar surgeon performing their 190th case, demonstrating that they are exactly where they should be at this stage of training with a very nice capsulorhexis. While the surgical skill is excellent, this case prompts an important point of reflection regarding international training timelines: a full ophthalmology residency in the USA is completed in just 4 years. If your UK training pipeline requires 7 or 8 years to reach the same level of surgical competency, it raises the valid question of whether registrars are being utilized for “cheap labor” to staff government health facilities rather than having their education prioritized. What are your thoughts on the stark differences between the highly structured 4-year US residency system and the extended 7-to-8-year training tracks seen in the UK and other systems? Please comment your opinions and regional training experiences below.
