
Yesterday’s video featured a case where the incision was too small and the IOL became stuck. Today’s video is me operating where the incision is just a little too small to insert the IOL and that is because this patient has prior radial keratotomy (RK) which means our cataract surgery incision must be carefully squeezed between these RK cuts without intersecting them. And on top of that, this patient has very flat keratometry from the RK which means the IOL power is very high at 29.0 diopters. This means that the optic of the IOL is thicker and that it requires the larger C cartridge (instead of the smaller D cartridge) and that needs a larger incision. So what should you do?
