2522: How should you remove this plaque?

This patient had a prior pars plana vitrectomy for posterior segment disease and now presents to you with a cataract a year or so later. After remove the nucleus, while you are clearing the cortex, you notice this posterior capsule plaque. It is adherent to the posterior capsule and it proving challenging to aspirate. How would you approach this case? Our guest surgeon does a posterior capsule opening but then when the IOL is inserted into the capsular bag, there is an issue… Great learning case that I know you’ll enjoy!
