
This patient developed severe capsular bag phimosis over the years and this has resulted in a dislocated IOL. Looking at this picture, what kind of IOL was it? Where is the IOL now? And how will you fix it? The original IOL was a plate haptic lens where the IOL has the shape of the capsular bag seen in the photo. The first toric monofocal IOLs that we had in the USA were of this design, being single-piece plate haptic IOLs made of silicone. With capsular contraction, performing a YAG capsulotomy could result in posterior dislocation of the IOL into the vitreous cavity. Fortunately, we rarely use these IOLs now. The surgeon also shows an innovative way of fixating a single piece acrylic IOL (not the typical three-piece using Yamane ISHF). Please comment below.
