2404: the probe just punctured the capsule

For experienced surgeons, complications involving the capsule happen more often during epi-nucleus and cortex removal than during nucleus removal. In this case, the nucleus removal goes smoothly with a nice chop technique. Then when it comes time to aspirate the epi-nuclear shell, the phaco probe punctures through and hits the posterior capsule. The posterior capsular opening extends during surgery as vitreous prolapses. How do you prevent this complication? And if it happens, how do you successfully recover from it?
