1933: cannula punctures posterior capsule

This patient started with an intumescent white cataract and that itself is a challenge. However the real difficulties happened at the end of the case when the OVD cannula punctures the posterior capsule. Fortunately there is no remaining cataract material to remove and the only remaining task is to insert the IOL. But can it be that simple? Or will an anterior vitrectomy be needed? Should you place a single-piece IOL in the bag? Or a three-piece IOL in the sulcus?
