
This patient has extensive zonulopathy and phacodonesis which is going to make the cataract surgery difficult and also pose a challenge for securing the IOL. The surgeon uses hooks to support the capsule, along with a CTR, while performing phaco and cortex removal. With an empty capsular bag, two Assia Anchors are used to fixate the capsular bag to the sclera using 6-0 prolene and the flange technique. These anchors have multiple holes for fixation and other variants of suturing can be done as well. This is a beautifully performed case and there are some great learning points.
