When a patient develops a white cataract right after an intra-vitreal injection, you can be pretty sure that the capsular bag was violated. This patient also developed high intra-ocular pressure (IOP) from the injection and then an anterior chamber tap was performed however that resulted in iatrogenic iris damage. Just think about how precise we have to be as ophthalmologists (sub millimeter) and how much our services have been devalued. This is going to be a tough case. What would be your approach knowing these challenges ahead of time?
