
The idea of a lens snare or loop to mechanically split the nucleus is decades old and we previously discussed it in this post: link here. The idea was also used by others to create the miloop device which is being used in this case. However, instead of just splitting the nucleus, the lens loop also severely damages the capsular bag. Watch the video carefully and leave a comment below describing when you think the posterior capsule broke and how you would manage the case. This is a very tough case by an experienced surgeon and it just goes to show you how stressful cataract surgery can be.
