Opacities seen during cataract surgery

In this unusual case we see opacities in the red reflex during cataract surgery. Most of these are within the crystalline lens and will be removed during the procedure. But there are still some stubborn spots that we simply cannot aspirate with the probe. Why not? What are these spots?

  1. These spots really dont matter in a normal papillary aperture it can usually be seen during retro illumination and it had no effect on vision
