
Reaching the milestone of 1,000 cataract surgeries represents a transformative shift from technical proficiency to surgical fluency. By this stage, a surgeon has moved past the steep initial learning curve, developing the muscle memory and tactile feedback necessary for fluid phaco. The anxiety of early training is replaced by a nuanced understanding of tissue behavior and fluidics. However, case 1000 is also a humbling realization of how much remains to be mastered. While the routine cases feel predictable, the next thousand cases shift the focus toward managing complex pathologies like brunescent nuclei, zonular laxity, and small pupils, where judgment becomes as critical as manual skill. It marks the transition from being a competent surgeon to becoming a true specialist who can anticipate complications before they arise. Great job – and I have a great bit of advice for this surgery.
