The light pipe, traditionally used in vitreoretinal surgery, offers a unique advantage when employed intraoperatively for this purpose. By introducing the light pipe through a paracentesis, surgeons can achieve oblique, tangential illumination of the capsule. This enhances contrast and depth perception, especially in cases where the red reflex is poor or the capsule is opacified. The directional lighting reveals subtle capsular folds and residual lens epithelial cells that may be missed under coaxial microscope illumination alone. This technique is particularly valuable when managing pediatric cataracts, posterior capsular opacification, or after anterior vitrectomy. Moreover, the handheld nature of the light pipe allows dynamic angling, further improving visualization without increasing intraocular turbulence. When used judiciously, it significantly enhances surgical precision and safety during posterior capsule polishing.
