
This video showcases an advanced technique (not for beginners) to perform a capsulorhexis in eyes with white cataracts. The technique is to decompress the capsular bag and then using gentle suction with the phaco probe to hold onto the anterior capsule to create a mini-capsulorhexis. The double capsulorhexis technique is then performed to safely enlarge the capsular opening to the desired 5 mm diameter. The key is to have the phaco machine setting in Irrigation/Aspiration mode (not phaco mode) so that no ultrasonic energy is delivered. Then using very light suction, the capsule can be grabbed and controlled. Very innovative but it can be tricky.
