We all love the fun CataractQuiz™ cases and in this video we are also asking for a crowd-sourced answer to determine, when did this capsule break? Our guest surgeon is talented and experienced but the capsule rupture here was so subtle that I had to watch the video a few times just to make a guess as to when it happened. Later in the case when the IOL is delivered, it becomes very obvious that there is no posterior capsule support. Watch the video carefully and in the comments leave the time at which point you noticed the capsule rupture. As a group, we can all help each other and learn together!
My be the capsulae ruptured during routation of last peice of nucleus it was routated without sufiecient space posteriorly
I think it broke during phacoemulsification with the phaco tip, when the epinucleus was suddenly aspirated in the center, and the tip was too near the posterior capsule. That was near the end, and the phaco tip was abruptly removed. When I finish the nucleus removal stage, I always insert a certain amount of OVD before removing the phako tip out of the eye. Even if it was an uneventful nucleous removal according to me. Enrique J Corral MD
