This patient sustained an ocular injury which resulted in a traumatic cataract with zonulopathy. In these cases, my first step is usually to inject a small aliquot of triamcinolone to see if there is vitreous prolapse present. In this case, when the trypan blue dye was injected, it went through this large zonular gap and leaked into the vitreous cavity. Now the red reflex is gone. So what is your next move?
