2559: two cases of fixing zonular defects

Our guest surgeon has submitted two cases of fixing zonular defects during cataract surgery. In one case the zonulopathy appears during IOL manipulation and the other case (title slide picture) happens during aspiration of cortical material. How many clock hours of zonular loss can be address with a capsular tension ring (CTR)? Three for sure, but what about 5-6 clock hours as seen in the title picture? When do you need to suture in a capsular support segment? Please give us your best advice and pearls in the comment section below.
