“It’s not over until it’s over” is a saying that fits this case particularly well. Our anonymous guest surgeon is an experienced and talented ophthalmologist who did a beautiful job with this tough case of a small pupil cataract surgery. At the very end of the case, with the IOL in the capsular bag, the last step was to remove the viscoelastic using the bimanual IA probes. Because the iris was still tented up by the iris hooks, the aspiration probe ends up catching the iris and then causing an inadvertent iridodialysis which then results in a small hyphema. What should you do now?
