2347: fixing acute angle closure glaucoma

Our guest surgeon, Professor Ramesh Ayyala from USF Tampa, demonstrates how he fixes acute angle closure in a complex case. This patient has cornea edema and no iris visible, so doing a laser peripheral iridotomy is not likely to provide any useful benefit. Instead the patient is brought to the operating room and the issues are surgically addressed. This starts with doing cataract surgery which will restore anterior chamber depth, then synechiolysis is done by pulling the iris out of the angle, and finally a pupilloplasty is performed. Truly a tough case with a great outcome. What is your approach to these tough cases of angle closure glaucoma? Please comment below.
