
We have all had cataract cases where some posterior capsule lens material remains adherent. We can try to aspirate it with the IA probe but when we see the capsule striae created as that 4 micron thin tissue is pulled into the suction port, we get uneasy and fear that a capsule rupture could occur. Some surgeons just leave this central deposit of lens material and perform a YAG laser capsulotomy in the post-op period. In this video, our guest surgeon has a different idea: use balanced salt solution (BSS) on a 27g cannula to power wash the posterior capsule.
