In this tough case the patient has a visually significant cataract but also a congenital coloboma which you can see in the partial maldevelopment of the crystalline lens and the quadrant of absent zonular support. Our guest surgeon does a great job of performing the cataract surgery and then using a Cionni ring to secure the bag-IOL complex to the sclera using polypropylene suture and the flange technique. How would you approach this tough case?
