
What we call viscoelastics are now referred to as OVDs: ophthalmic viscosurgical devices, in some part because we can then classify them as devices instead of pharmacologic agents. Regardless of the name, this jelly-like substances can be used in ophthalmic surgery to protect tissues, create space, and pressurize. But there is a delicate balance since over-inflation with OVD can cause force on delicate tissues which could result in a complication. In this video, an over-inflation of OVD causes a capsulorhexis to radilize and run out. This now must be managed carefully to prevent further capsular compromise. This is such an important lesson for any surgeon who has done less than a few thousand cases. You have been warned! Watch carefully!
