2133: surprise true capsular exfoliation

This is a very rare case that I have never encountered in my career: a true capsular exfoliation where there are effectively two anterior lens capsules. This video is a must watch so that if you encounter it in the future, you will know how to manage it. Historically, true exfoliation of the anterior lens capsule was seen in glass-blowers due to the extreme heat exposure. And we call our commonly seen condition pseudo-exfoliation since it is not true capsular doubling.

  1. Have never seen this in 30 years. Great case! A cystotome rather than a forceps at initiation of the rhexis would have been an interesting try here-
