This patient complains of “multifocal misery” as well as extensive floaters which can further degrade the image quality. An IOL exchange is performed to replace this trifocal IOL with a monofocal IOL and at the same time, a pars plana vitrectomy is done to remove the vitreous floaters and debris. And to avoid needing to do a YAG laser capsulotomy in the future, the vitrector is used to clear the central posterior capsule. Great case!
