2063: the problem of exposed flanges

We have featured many techniques of IOL fixation in cases of insufficient capsular support such as the Yamane technique, the belt-loop technique, and Gore-tex fixation, and with all of these the key factor for long-lasting IOL fixation is ensuring that there is no erosion through the conjunctiva. This can be accomplished by making sure that the flanges created on the IOL haptic tip (Yamane) or on the polypropylene suture end (belt-loop) are buried completely within the sclera. If you see the blue plastic flange sitting under the conjunctiva then it will likely erode and extrude in the future. This video shows our guest surgeon Dr Steven Safran doing a re-operation after a failed belt-loop procedure. He also discusses these cases with me in detail during our podcast together which will be featured tomorrow. Be sure to tune in!
