This patient had a terrible case of floppy iris syndrome and iris prolapse during the initial cataract surgery which was months prior. With a delicate blue iris, the stromal tissue is more prone to iatrogenic damage and the patient ended up with a large iris defect. This is a complication that has happened to all cataract surgeons and it is largely due to the patient’s anatomy and tissues. If this has not yet happened to you, then you are still at the beginning of your career and trust me, it will happen. While we have videos about fixing the iris prolapse, sometimes even the best techniques fall short of full success. This video shows how to suture this defect and achieve a great functional and cosmetic outcome for the patient. Iris suturing pupilloplasty is done with 10-0 polypropylene on a long CIF-4 needle.
