Time for a Cataract Quiz: Look carefully at this title photo and ask yourself, “Why is this clear crescent visible in the red reflex?” Hmmmm, this is a young patient (30s) with a history of facial / ocular trauma more than 10 years ago, but with development of this unilateral cataract over the past year or two. The other eye is perfectly normal with zero cataract development and emmetropia with 20/20 vision. At the pre-op consultation we did not detect any obvious phacodonesis but we did see this gap. What is your approach to this case? What is the zonular status? What tools or devices do you want to have available in your operating room before starting this surgery?
