The challenge with an intumescent white cataract is completing the capsulorhexis without having capsular run-out (Argentinian flag sign). This is because the white cataract cortex has liquefied and is causing a high intra-lenticular pressure. We have so many videos of how to help minimize this risk and you should be doing your homework to make sure that you understand. If you try to complete your regular capsulorhexis, then you will have a capsular run-out just like in this case where it happens once and then twice… how do you recover now?
