
This patient had a prior history of anterior uveitis which resulted in the formation of synechiae from the iris to the anterior lens capsule, resulting in a distorted pupil. While it is relatively easy to break the synechaie and separate the iris from the anterior lens capsule, this may not fully restore the iris to a normal configuration. There is often a fibrotic membrane which has grown in this space and if we carefully peel it off, then we can help ensure a better anatomic and cosmetic outcome. This video gives my best pearls for accomplishing this without causing damage to the iris root.
