Our guest surgeon, Dr Brandon Ayres from Wills Eye Hospital, gets referred a high volume of complex anterior segment reconstruction cases and showcases them on his YouTube channel. This video shows his technique of doing an IOL exchange for a patient with a very dislocated toric IOL. In order to secure the new IOL in position, he performs a posterior capsulorhexis and then captures the optic in order to give long term stability. This video has some great pearls including how to dissect out the old IOL, how to perform the posterior capsulorhexis, and what adjustment may be needed to the IOL power.
