It seems like a routine cataract case, nicely performed with a phaco chop technique, but there is something more. Look carefully at the picture here and ask yourself if you notice anything hiding? The surgeon is about to implant the IOL but then he notices something… If you cannot figure it out then you may need to return to the operating room.
