1540: Why won’t the IOL stay in the bag?

Cataract Quiz: Look at the picture above and see if you can figure out why the IOL keeps coming out of the capsular bag. The video is not the highest quality, so the photo above is good but not great. Is the issue the capsulorhexis? Is there something in the posterior segment? Is the IOL not fully in the capsular bag? Is it related to the retrobulbar injection? Does the patient’s body habitus play a role? What is going on?

  1. I’m not entirely sure that the bottom left haptic (trailing haptic) is in the bag. I tend to look for the rhexis edge to overlap the optic at the haptic-optic junction and I never see that for the bottom left haptic. It may have been helpful to rotate the IOL more to ensure that it was completely in the bag. The surgeon may have been more confident due to the 3D view through the operating microscope. Thanks for sharing this interesting case!
