
Beginning residents are amazed when they see that a complete capsulorhexis can be completed with just a cystotome. Using just a bent needle, the surgeon can create the entire capsulorhexis through a small paracentesis incision. The technique involves using the slight friction from the needle tip to the capsule to bring it along in a circular pattern for the full 360 degrees. This video shows that technique in the setting of a white cataract. This is something that all surgeons should learn: if you are now starting your capsulorhexis flap with a cystotome and then switching to forceps, why not try doing 90 degrees, then 180 degrees and then finally 360 degrees with just the cystotome?
