
It is easiest to insert the capsular tension ring (CTR) when the capsular bag is completely empty of lens material. If we insert the CTR while there is still cortex present, the CTR can trap the cortex at the capsular bag equator which makes removal challenging. In this case the surgeon felt the need to insert the CTR prior to removing all of the cortex and he shows the approach to successfully overcome this challenge. As the saying goes, insert the CTR as early as you need it, but as late as you can.
