
Cortical lens opacities tend to block the red reflex which can make capsulorhexis creation more difficult. Since these cortical spokes are often liquefied lens material, you can actually move them out of the path of the capsulorhexis and improve your visualization. Look at the picture here and compare the before and after results. I was able to push the liquefied lens material centrally and remove the cortical spoke from the intended capsulorhexis path. Note that this is best performed prior to puncturing the anterior lens capsule.
