
This patient had cataract surgery years prior with significant intra-operative complications. The posterior capsule was ruptured and a sulcus IOL was placed but without removal of the cortex. This caused a dislocated IOL and extensive Soemmering ring formation along with fibrosis and opacification of the residual cortex. A YAG laser was used in an attempt to clear the visual axis but this ended up simply pitting and damaging the IOL optic. Fortunately, the patient found Dr Singh who performed an IOL exchange along with removal of the Soemmering and cortex opacities. The patient achieved a beautiful surgical result.
