Choosing Toric IOL Steps

For toric IOLs, we need to select the best spherical power as well as the ideal toric step. What is the effect of WTR (with-the-rule) versus ATR (against-the-rule) astigmatism? What about the effect of the posterior cornea — should we measure it or use standardized values? Does placement of the phaco incision affect the outcome? What if the ideal toric power is between two steps? What if the toric IOL is not perfectly aligned with the corneal astigmatic meridian?

30 degrees of misrotation will cause loss of all toric power. This means that about 33% is lost for every 10 degrees.
