In cases where the capsulorhexis is small or where the nucleus is particularly large, it can be difficult to bring large nuclear pieces out of the capsular bag with just the vacuum suction of the phaco probe. In these cases, using the chopper can be helpful to lift these pieces.
In this case our patient is very myopic with a large anterior segment. The white-to-white measurement is 13 mm in diameter and the capsulorhexis is well-sized at 5.4 mm in diameter. Due to the large anterior segment, the patient also has a large crystalline lens.
