Quiz: Why is the posterior capsule wrinkled?

This seemed like a totally routine case. Good incision, good capsulorhexis, efficient nucleus removal, and complete cortex removal. There was no evidence of weak zonular support or capsular issues during the surgery. When the IOL was injected into the capsular bag, everything looked great. But then, at the end of the procedure, the posterior capsule developed wrinkles in the center of the optic. Why?
