When the capsulorhexis runs out toward the equator, immediate salvage maneuvers can successfully rescue the capsular integrity. Re-establishing high-viscosity viscoelastic will flatten the anterior capsule making further manipulation easier. The surgeon can gently grasp the runaway capsule flap, direct the tear centrally using the Little maneuver. Securing a stable capsular rim ensures safe phacoemulsification and allows for reliable in-the-bag intraocular lens placement. Watch the video to see how this capsulorhexis tear extension is successfully managed and rescued. What is your go-to maneuver when a capsulorhexis runs out to the periphery?
