3032: IOL Scaffold Techniques – 5 minute review and compilation video

Collage of images demonstrating the IOL scaffold techniques in cataract surgery, showcasing steps for IOL implantation and management of posterior capsule rupture.

The IOL scaffold technique is one of the most powerful bailout maneuvers in anterior segment surgery, offering a brilliant mechanism to prevent nuclear fragments from falling into the vitreous cavity when a posterior capsule defect occurs. By injecting the IOL directly beneath the remaining nuclear pieces the optic acts as a physical barrier that seals off the posterior segment. With the vitreous space protected and any present vitreous cleared with an anterior vitrectomy, the remaining nuclear and cortical material can be safely emulsified or aspirated above the artificial floor without risking posterior dislocation. Watch this high-yield compilation video to review four distinct clinical scenarios, including combined iris and IOL scaffolding, where this essential technique saves the day during some tough cases. What is your preferred technique for managing remaining nuclear fragments after encountering a compromised posterior capsule? Please comment your pearls below.

  1. The management of pcr depends on the step of the surgery that happens but 3 targets must be achieved:
1- prevent cristaline material from going into the vitreous
2- apropiate manage of present vitreous
3- implantation of an adequate IOL
