Performing complete cataract surgery through a 2 mm pupil when the cataract is 9 mm in diameter requires an incredibly bold and brazen approach. Operating through such extreme miosis significantly limits visualization, making it nearly impossible to be entirely certain that all peripheral cataractous lens material has been cleanly removed and to ensure that both IOL haptics and the optic are securely positioned inside the capsular bag.
