What are your surgical strategies when managing a cataract in an eye with microspherophakia? In this video, our guest surgeon demonstrates the management of a crystalline lens that has a significantly smaller equatorial diameter and an increased anteroposterior thickness, giving it a characteristic spherical shape. Crucially, the surgeon notes that there is no significant zonulopathy or generalized lens instability present in this case. Instead, the primary surgical challenges stem from the unusual lens morphology, a shallow anterior chamber, and the altered fluidics dynamics that come with a smaller lens diameter.
