2949: Opaque cornea with a surprise inside

Operating on an eye with an opaque cornea is challenging enough, but sometimes there is a massive surprise waiting for you inside. Our guest surgeon is Professor Ramesh Ayyala from USF in Tampa, Florida and he presents a mind-blowing case involving a patient with glaucoma, a failed corneal transplant, and a completely dropped cataract nucleus sitting right on the macula. Because you cannot visualize the posterior segment through the scarred cornea, a B-scan ultrasound is critical to making the diagnosis before making an incision. This video demonstrates the strategic planning and careful surgical steps required to manage such a complex anterior and posterior segment disaster. It shows that even when facing a worst-case scenario, following a logical, step-by-step approach allows you to successfully navigate the case. I may never see a case like this in my own clinic, but it sure is fun to watch and learn.
