
Anterior segment reconstruction in an eye with an opaque cornea and dense cataract is one of the most complex procedures in ophthalmology. The surgery begins with a penetrating keratoplasty. Once the diseased corneal button is removed, you finally have an open sky view of the internal structures. First, you perform iridoplasty to lyse extensive synechiae and restore the iris plane. This is followed by the cataract removal, which must be done with extreme care to manage the lack of posterior pressure support. After the lens is extracted, a stable intraocular lens is placed in the sulcus. The final step is the pupilloplasty, where you use fine sutures to create a central, regular pupil. This reconstruction of the iris helps minimize future glare and improves the quality of vision.
