
Let me first start out by telling you that I do not know exactly what happened here and we are counting on the crowd-source, group knowledge to solve this mystery. This patient has a cataract without any prior history of trauma. No prior surgery either. And no prior intra-vitreal injections. Then during a seemingly normal cataract surgery the nucleus drops into the vitreous cavity. Why? Of course there is an issue with the patient tissue and protoplasm, but what else?
