Instead of creating a peritomy, the surgeon fashions small lamellar scleral pockets beneath the intact conjunctiva, allowing the suture knots to be buried securely and reducing postoperative irritation and scarring. This method provides strong scleral fixation and preserves healthy conjunctiva for future glaucoma or retinal procedures. During lens fixation the technique facilitates careful centration because the pockets provide stable, symmetric anchoring points that help control the lens position once the sutures are tied.
