2747: metallic foreign body induced cataract

Traumatic cataracts resulting from a penetrating injury, such as this case of a metal wire perforating the cornea and lens capsule, require careful surgical management to restore vision and preserve ocular integrity. Preoperative assessment includes slit-lamp examination, anterior segment imaging, and evaluation for associated injuries, including corneal laceration, iris damage, and vitreous prolapse. The corneal laceration is first repaired, ensuring watertight closure and structural stability so that the lens removal can be done in a controlled and stable chamber with good fluidics. Anterior capsular tears are assessed, and careful phacoemulsification or lens aspiration is performed to remove the traumatized lens material. In cases of capsular rupture, a sulcus-placed intraocular lens may be required. Postoperatively, anti-inflammatory and antibiotic therapy is instituted, and careful monitoring for secondary glaucoma, retinal complications, or corneal decompensation is critical to optimize visual recovery.
