After performing 4,500 cataract surgeries, a surgeon is expected to have reached a high level of technical proficiency, confidence, and efficiency. At this stage, the surgeon should be able to handle routine and complex cases with consistency and excellent outcomes, including small pupils, pseudoexfoliation, weak zonules, intumescent lenses, and posterior polar cataracts. Complication rates should be low, and the surgeon should be adept at managing intraoperative challenges such as posterior capsular rupture or zonular dialysis. Surgical times are typically reduced without compromising safety or precision. Beyond technical skill, the surgeon should demonstrate sound clinical judgment in preoperative planning and postoperative care, as well as the ability to teach and mentor others. Mastery of various techniques, including phacoemulsification and manual small-incision cataract surgery, is expected. At this level, many surgeons contribute to innovation, training, or research, further advancing the field of cataract surgery and improving patient outcomes.
