2650: Explanting a segmented ClearView 3

The Lenstec Clearview 3 multifocal intraocular lens (IOL) is designed to provide vision at multiple distances, often reducing the need for spectacles. However, some patients experience visual disturbances such as glare, halos, decreased contrast sensitivity, or persistent dysphotopsias. These symptoms, especially in night or low-light conditions, may be intolerable despite good visual acuity, leading to dissatisfaction and necessitating lens explantation.

Explanting the Clearview 3 IOL requires careful surgical planning. Ideally performed within the first few months post-implantation (before significant capsular fibrosis), the procedure begins with viscodissection to gently separate the lens from the capsular bag. If the haptics are fibrosed into the capsule, careful dissection should be performed. The lens is then dialed into the anterior chamber or bisected in situ using micro-instruments and removed through an enlarged corneal incision. Capsular integrity should be preserved to allow implantation of a monofocal IOL, often in the bag or sulcus depending on intraoperative findings. But in this case, the patient decided to have a different lens design, the PanOptix Pro, implanted. Is the patient going to be happy?
