2619: IOL exchange with an open capsule

We recently featured a video where I explained some patient situations where a multifocal IOL could be problematic. One of those situations was electing to perform refractive lens exchange for a patient without cataracts to address presbyopia. This is what happened in this case where a 50ish year old patient sought treatment for presbyopia and a refractive lens exchange was performed on this one eye. Immediately the patient was able to now read up close without glasses but the quality of vision was just not as good as the other phakic eye (while using reading glasses). The patient tried to adapt to this new vision and the operating surgeon even performed a YAG laser capsulotomy thinking that may help maximize the vision. After more than a year of discussion with me, the patient finally decided that he wanted the multifocal IOL removed and replaced with a monofocal IOL. This procedure is far more challenging because the posterior capsule is open and an anterior vitrectomy will be needed. This video shows my technique of successfully performing this tough case. The patient ended up very happy.
